Waiver

I understand and acknowledge that the training, programs, and events held by Renzo Gracie Niagara and/or Muay Thai Niagara may expose me to many inherent risks, including accidents, injury, illness, or even death. I assume all risks of injuries associated with participation, including, but not limited to, falls, contact with other participants, and the effects of the weather, including high heat and humidity. I know and appreciate all other such risks. 

I acknowledge my responsibility to communicate any physical and psychological concerns that might conflict with my participation in the activity. I say with all certainty that I am physically fit and mentally capable of performing the physical activity I choose to participate in. The participant understands that in case of injury, first aid is the only medical treatment Renzo Gracie Niagara and or Muay Thai Niagara will provide. 

After having read this waiver and knowing these facts, and in consideration of the acceptance of my participation and Renzo Gracie Niagara and or Muay Thai Niagara furnishing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE Renzo Gracie Niagara and or Muay Thai Niagara, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in  Renzo Gracie Niagara and or Muay Thai Niagara classes, training, programs and events related to Brazilian Jiu-Jitsu, Hatha Yoga, Power Yoga, Forro Dance and every type of activity provided by Renzo Gracie Niagara and or Muay Thai Niagara.

I agree that any pictures, audio, or visual recordings taken of them in connection with the class can be used for publication, promotion, articles, shows and advertisements without additional consent and without compensation at this time or any other time.

I understand this Waiver and Agreement and agree to its provisions. I know this is a waiver and a release of liability, and I voluntarily agree to its terms. I am not under the influence of any drugs, alcohol, or other intoxicants. I am not suffering from any illness or incapacity. I am over 18 years of age. (If not over 18 years of age, a parent or guardian must sign). If I am a parent, and I'm signing up for my child, I agree with all the information similarly.

I have read and understand Renzo Gracie Niagara's and or Muay Thai Niagara's terms of service.

If this waiver is delivered by any electronic means, said delivery shall be of the same force and effect as the delivery of an original signature.
